When merging two books together and one author now has no books, delete it.

When downloading a book, we use the filename in the filesystem, which doesn't get updated when a book is edited with books edit.
Two possible solutions:

1. Update the filename on save.
2. Generate the filename when downloading a book.

I prefer 2, because it won't break if we have a duplicate of something somewhere.

Saving a file twice in books edit after changing metadata for the second time gives an error.

Authors and titles for imported books should be searched for case-insensitively.
