Replace reference to AutoMapper.Extensions.Microsoft.DependencyInjection 12.0.1

AutoMapper.Extensions.Microsoft.DependencyInjection 12.0.1 has been deprecated so 12.0.0 should be used to not strictly reference AutoMapper 12.0.1.

Health checks fail when AddDbContextFactoryWithDefaults is called multple times with the same DbContext

When AddDbContextFactoryWithDefaults is called multiple times with the same DbContext type, multiple instances of the DbContextFactoryMigrationHealthCheck and DbContextFactoryConnectionHealthCheck are registered with the same name.
This leads to failures in the health checks.

Provide an alternative to StartupBase for the minimal hosting model

When using the newer minimal hosting model there is no startup class anymore.
There should be alternatives providing extension methods for IHostApplicationBuilder.

This is needed to provide native support for .net Aspire.
