There is an issue with docGenerator.js on line 57, not replacing special character used for path-to-regex.

As only : is replaced, the generator crash if advanced functionality are used (such as optional (:{name}?), multiple (:{name}* and :{name}+), etc.)

My middleware is never called if the accesses fail.

The accesses are checked before the middlewares.

It should be the inverse, so that we could populate the ctx.state before going into the accesses.

It would be great if you could add some documentation about how koa-smart is handling errors and how to throw inside a route.
