edit pads on more than one server

The server url and api key parameters could be buffer local (or configurable as pairs) to enable editing documents from more than one place.

change hooks should be buffer local

the functions run as change hooks i.e. etherpad-before-change-function and etherpad-after-change-function should be specific to the minor mode and buffer local.
