On follow-up, Adverse Event column should force choice of “Yes”, “No”, or “Inconclusive” and “If Adverse Event: Severity?” column should force choice of “Mild”, “Moderate”, or “Severe”.

and honestly, thinking about the TH row, we don't need the visual cues there anyway.

After thinking about this one, I've relabeled it as a bug rather than an enhancement because as it currently stands, it is not correct.

The reason for the difference in my mind. "It works as I wrote it" so it's not a broken part, it's a change on how things are done.

Close but slightly off. "Yes/No/Inconclusive" should be in column that currently says "No/Change?". If AE, OMT Related? column should look like the others (Definitely/Probably/Not Sure/Unlikely/No).

That works. If you took out the "No" and "Change?" at the top, I think it would be perfect.

No, because we don't really have a way to say automatically on the follow-up questions whether it is a "Yes" or "No" since there's no severity change or something like that. So it really is that the EAC is deciding whether it is an AE or not, rather than changing from the algorithm-determined status.

Ah. They're not reporting it as an AE. They're just reporting that it occurred. So, they're reporting they had a visit to a doctor. It doesn't mean the patient thought it was an AE. It could have been for a wellness check, for example. That's why it's different from the symptoms.

All of the symptoms are "bad things" whereas a visit to the doctor might or might not be a bad thing.

But how did you know that it was a "Yes" to begin with? The EAC has to make a decision on every follow-up visit and I think it is less confusing for them if there are no "preconceived ideas".

I used "Adverse Event?" as the visual header, maybe that's more clear?
