not compatible with TF 0.12+, only works on TF 0.11

Running the convex.py example with latest tensorflow fails, but works with scs solver.

tried updating to handle TF 1.1 under Python3 v3.6 on Windows

changes "concat" arg order, "reverse" axis list, "pack" -> "stack" and "mul" to "scalar_mul" were necessary
